This achievement is for The Bard's Tale II: The Destiny Knight. Although it says "Visit each city.", it only means for Bard's Tale II, and doesn't include Skara Brae or any of the BT3 cities.

You will (and must) visit each city if you play the game through, so this is unmissable and inevitable. There are six cities that need visiting:

    Ephesus - N40 E15
    Colosse - N32 E23
    Tangramayne - N23 E4
    Philippi - N15 E15
    Thessalonica - N7 E24
    Corinth - N6 E4

I take no credit for, but it will help for this.

Note: Weirdly, even after visiting all 6 cities, the achievement didn't pop for me until I saved and loaded a game in this state. YMMV.

EDIT: Thanks to funkyTii and Villa I for the comments: This one's definitely glitchy for when it pops. If saving and loading doesn't work, try going to Saradon's (possibly requiring entering the dungeon and leaving again.)

funkyTiiWeirdly, it only popped for me after visiting Saradon’s house. At first i thought this was glitching on me. Did a reload with all visited cities and moved on. Try going to Saradon and leave.

Villa IIt also didnt pop for me at first after visiting all six cities.
Saving and loading didnt do it for me.
So i went to Saradon's, entered the house without ascending the stairs, just like i did when going for the "A Fallen Hero" achievement, didnt pop again. Entered Saradon's again and this time actually entered the dungeon and instantly left. Shortly after it unlocked.

HeptagrammatonSame experience as the above commenters.

When I went to the only city I hadn't visited previously, Colosse, I was expecting the achievement to pop but it didn't. Went on a quick tour revisiting all the other cities but that didn't help.

Came here and read what funkyTii and Villa I wrote above and tried entering Saradon's House (and ascended the stairs into the workshop proper). At this point the achievement unlocked. (Edit: I just want to note for anyone reading this that I had previously been to Saradon's Workshop and explored it completely).

Thank you Malkoris for the solution and the other guys for sharing their solution to the glitch.
