As of 12/09/2019 this achievement seems to be fixed as the collectibles were glitching out for people at release where they would not register properly.

Here's a complete list of the the collectibles throughout that game with notes detailing where they are located.

Act One – Chapter 1
00:10 - Weathered Specticle: This can be found directly behind you on a rock when you take control of JD.
00:34 – Onyx Guard Challenge Coin: Go up the path from where you spawn initially and stick to the left and you’ll find it on the floor.
00:54 – Wellness Advisory Poster: Continue through the level as normal and you’ll get to a rest room and it’ll be on the left wall.
01:23 – Cog Tags: Fallen Azura Gear: This is in the area after you defeat the Carrier you will head on inside and Baird will speak to you. It will be in the room across the hallway just before the stairs.
01:45 – Chariman’s Wristwatch: Head up the stairs from where you just were and there will be a room, head in that room and turn left and you’ll see a small room and it will be in there on the table.
02:07 – Lightmass Missile Strike Authorisation: Continue on as normal until you get to the room where either your DB Protector or Guardian get’s destroyed by the Scion. Take care of all the enemies that are there and stick to the left side and you’ll see a small room with some stairs, head up there and you’ll find it on the table.
02:31 – UIR Hammer of Dawn Blueprints: Just from the last collectible you’ll have to open a door by using Jack, head through the door and it will be on the table directly in front of you next to the Globe.

Act One – Chapter 2
02:48 – Photograph of Kilo Squad: This will be in the room that you spawn in on the desk on the left side near where Baird stand initially.
03:08 – Trauma Record Marcus Fenix: Head up to where Baird tells you to pick up the component for Jack and it’ll be on the left side of the room on a box.
03:29 – Mark 3 Lancer Blueprint: It’s in the same room and the Trauma Record but it’s on the left side on another box.
03:44 – Gym Notice to Trainees: After you meet Fahz for the first time turn around and it’ll be on the wall next to the vendor machine.
03:57 – DBI Marketing Brochure: Moving from the training room go down the corridor and turn left and it will be on the seat.

Act One – Chapter 3
04:19 - Cog Tags: Fallen Raven Pilot: After you defend yourself and Cole whilst you are stuck under the Raven walk down the corridor and it’ll be on the floor.
04:31 – The New Ephyran: 20th Anniversary Edition: Directly in the next room from the previous collectible and it will be on the desk in the far-left corner.
04:45 – Seditious Literature: When you first encounter the Swarmak where it will destroy the wall in front of you. Head up the stairs, turn left and it will be on the desk in the corner.
05:03 – Settlement 2 Welcome Package: Continue on as normal until you get to this room where there are civilians and fellow Gears inside, and it’ll be in the small room on your left by the overturned table.
05:19 – Storied Embry Star: Continue on through the level until you meet up with Lizzie and defeat a Pouncer, in this area look for the building that’s left of the gate you have to go through and head up the stairs and it will be in the filing cabinet.
05:45 – Cog Tags: Fallen Gear at Condor Crash: Continue as normal and you will go through a crashed condor and it will be on the floor just before the deactivated DB’s.
06:01 – Seran Colosus: Get to the area where there are some more Juvies with grenades on them and it will be in the shop at the end of the road by the green car, head on inside and it will be on the table on the right as you enter.

Act One – Chapter 4
06:20 – Memo to Bomha Hotel Staff: Head through the level as normal until you get to this small gate you have to break open, head left and jump up the small ledge and it will be on the crafting table in the small room.
06:39 – Cog Tags: Fallen Gear at the Hotel: Head through the blue double doors and you’ll be in a dark room, it is directly in front of you on the floor next to some boxes.
07:01 – Lost Horse Plush: Continue through the chapter until you get to a door and once you are through there will be some stairs on the right side along with an archway in the middle, head through the archway and it’s in the left corner.
07:31 – Settlement 2 Birth Pamphlet: Head up the stairs from the room where you got the Lost Horse Plush and it’ll be in the dinning table on the left.
07:44 – Song Lyrics: Make your way to the theatre, head up the stairs and take your first left and you’ll find it in a small room on the table.
08:15 – Worn Lighter: Continue moving down the small corridor from the Song Lyrics and it will be on the table on the left side as you exit.
08:35 – Octus Canon Scroll Prop: When you get the stage where you have to pull the curtains it’s on the opposite side from where you are coming from in the far-right corner on a box.

Act Two – Chapter 1
08:57 – Hand Carved Riftworm Flute: At the start of this chapter head down the stairs and it will be in the room on your right on a table on the left side of the room.
09:18 – Ilima City License Plate: You’ll get to this area where you have to head down this road to progress, head down the road and on your right, it will be next to a furnace.
09:38 – Lost Gear Helmet: Head down the road a little longer and you’ll see a big giant pillar head past that, and it will be on the left side on the floor.
09:56 – Wooden Toy Lancer: Head through the first security gate and you’ll see this young person teasing you and it will be on the right side next to a concreate block.
10:15 – Makor Howl and the Battlin’ Ants!: Just past the Wooden Toy Lancer you’ll see a shop with a table of a boat inside and it will be in the left corner.
10:29 – New Village Rules Mandate: In the room on your left when you get to the area where two people are building stuff/arguing and it’s on the lockers on the left side.
10:43 – Note to the Chief: Continue a bit further and you’ll see this building that’s painted yellow after you’ve had your first hallucination, head on inside and it’s on the table.
11:00 – Norskos’ Letter: Continue on until you have to open the gate, before you do that bare right and it will be on a container.
11:16 – Outsider Skiff Schematic: When escaping from the village you’ll head to the room with the Silverback, it’s next to it on a box.
11:36 – Oscar Diaz Cog Tag: It’s in the room where you have to lower the Skiff to ride it, it’s in the far-left corner on a container.

Act Two – Chapter 2
11:51 – Grindlift Maintenance Notes: It’s in the Train Tunnel area so head inside take care of the Carrier there and it’s inside the train cart towards the back.
12:49 – Faded Photograph: This is north east of the Train Tunnel area where you just were. It’s inside a small train carriage nearby. Look for the disused train tracks.
13:12 – Lena’s Journal: This is in the Outsider Campsite north east from the Faded Photograph, head on inside and it’s next to the tents.
13:40 – Islander’s Lost Earring: This is south east of the Train Tunnel area and it’s next to a broken down Centaur on the floor.

Act Two – Chapter 3
14:04 – Intercepted Letter: When you get inside the New Hope Research Facility it will be in the right corner of the room on a ledge.
14:15 – Patient Analysis: AX-331-2: Continue moving on until you get to this dark corridor after you’ve fallen in a jail cell and it will be in the room directly in front of you on a table on the left side.
14:39 – Sovereign Directive 10345: In a locked room from just before the AX-331-2 collectible. Open this door with Jack and it will be inside.
15:00 – Patient Analysis: EV-184-9: Continue moving on until you get to the small room that’s light up green. Break through the window and it’s on the desk on the left side.
15:22 – Patient Analysis: RK-619-4: Keep moving until you get to a dark corridor with yellow painting on the wall, and it’s in the room on the right on the operating table.
15:41 - Timeworn Data Drive: Keep on making your way through the level until Kait start’s asking Dell what to ask Niles. You will then head down some stairs and in the next room the Data Drive will be on the right side of the room on the shelf.

Act Two – Chapter 4
15:55 – Security Memorandum: This is in a unmarked location that’s directly north of the East Tower Substation (really close by) and it’s in a small orange building in the left room in the right corner.
16:38 – Krav’s Locust Tag: This is outside the Old Derrick Site that’s in the North East corner of the map. Go to this area an it’ll be just outside on the left next to a broken down Grindlift.
17:06 – Grindlift Actuator Component: Head on inside the Old Derrick Site, get rid of all the enemies first then head left and jump up onto the broken down derrick and it’ll be inside the yellow box.
17:46 – Lena’s Scribbles: Head to the North Tower Substation follow the path for a bit and you’ll see an orange building with Lena’s Scribbles on the keypad.
18:07 – Orders From Ketor Skorge: Head to the other Old Derrick Site that’s in the middle of the map, and it will be on the right side on the floor past some ice boulders.
18:42 – Grindlift Radiator Component: Moving on from the Ketor Skorge collectible, head on inside the Old Derrick Site and take care of all the enemies that spawn. The Component will be on top the Derrick inside the yellow box.
19:16 – Letter To Momma: This is in an unmarked location that’s directly west of the Eastern Tower Substation you will find it next to a broken down Centaur and it will be on a box next to it.

Next complete the missions relating the Eastern and Northern Substations then go to the abandoned mine which is where the next set of collectibles are.

19:40 – Frayed Pouncer-Tooth Necklace: Whilst making your through this part you’ll have a battle on the ice that’s under a bridge whilst you are defending Jack to open the elevator. Once you are up the elevator you will see a ladder as you head from it and it will be up there.
20:04 – Nethercutt Mining Poster: When you get inside the mine itself, you’ll come to this room with some lockers in front of you. Head right and you’ll find the poster on the wall.
20:22 – Nethercutt Mining Journal: Continue through the mine until you get to this room with the Swarm Pods, the bear left and it will be in the room at the far back corner.

Act Two – Chapter 5
20:47 – Nethercutt Helmet: When you get to the door guarded by security turrets turn around and follow the path then turn right and it will be on the floor towards the back.
21:16 – Subject Analysis: UL-119-2: when you get inside you have to deactivate the ice for the first time, when you head down the stairs by the frozen Gear soldiers head up the stairs on the right side and it’s in the room on the right side.
21:45 – Subject Analysis: TE-872-3: when you get to the room after learning with the big yellow that’s in front of you. Turn around and it’s on the floor behind you.
22:07 – Lexigram Board: A bit further on Niles will ask you to deactivate some more turrets, the Lexigram Board is next to the yellow power box you have to disable.
22:27 – Subject Analysis: EV-184-9: After you shut down the place with Niles’ hand the collectible will be on the desk behind you in the left corner.
22:51 – Sketchbook: It’s in the room literally next door to the previous collectible, turn left and you’ll see it on the floor.

Act Three – Chapter 1
23:04 – Corisku Newspaper: At the start where you spawn, turn around and it will be on the bar behind you.
23:22 – Doomed Empire, By Logan Hobbs: This can be found down the stairs from where you are. Go across the room until you get to the baggage area turn right and it will be on the trolley.
23:44 – Missive To Agent 9: whilst inside the airport run past the security barriers turn right and it will be inside the room at the end of the corridor.
24:12 – Nomad’s Creed: Firstly you’ll want to talk to Baird to get the upgrade for Jack, then talk to Paduck. Go past Paduck and on your left will be the collectible on the table next to a tent.
24:30 – The Astounding Lightening Rangers: When you are on the runway by the Condor head to Hanger 4 and it will be on the left of the Helicopter.

Act Three – Chapter 2
All these collectible can be found in the Rocket Hanger area that you have to go to.

24:57 – Hammer of Dawn Command Case: When you get to the control room of the Rocket Hanger it will be on the right side in the right corner.
25:10 – OZP-Termination Directive: Heading from the command centre turn right go down the stairs, turn left and it will be in the far left corner on top some barrels.
25:25 – UIR Space Mission Brief: You’ll come to an area where there are some Swarm down below and two sets of stairs. Take care of the Swarm and go up the right set of stairs and it will be in the room towards the back.
25:59 – Loyalist Order from Major Toly: You will come to an area where you have to board another Gantry that is near some sandbags. Jump over the sandbags and the collectible will be behind it on the desk.
26:14 – UIR Tag Major Toly: When you get to the area where you see a tank directly in front of you after going through a gap in the wall. The collectible will be on the floor in front of the tank.
26:33 – The Major Speech: Towards the end of this segment you will go through a big crack in the wall and be brought to a room, inside that room is the collectible which is on the table.

Act Three – Chapter 3
26:47 – Nomad’s Sketching: This is in the Water Tower side mission area, kill all the Swarm. Head inside and the collectible will be on the table in the left corner near where the Nomad is hiding.
27:31 – UIR LC Circuit A1: This is in the City Ruins side mission area, kill all the Swarm until Jack opens the door. Head up the stairs and it will be on the desk.
28:10 – Well Worn Vasgari Flag: This is again in the City Ruins area, so from the last collectible go down the corridor and it will be on the wall on your right.
28:30 – UIR Tag: Fallen Loyalist: This is at the entrance of the Rocket Hanger, so go there and it will be outside the entrance on the right side next to a fallen loyalist.
28:57 – Loyalist Order to Lieutenant Melich: This is an unmarked location so it’s directly east of the bridge that you need to raise and it’s where the three abandoned tanks are located. Go there and it will be by the left tank on the floor.
29:11 – UIR LC Circuit C1: This is in the Artillery side mission area, take care of the enemies and Snatcher then head on inside. Go up the stairs and the C1 Circuit will be on the table in front of you.
29:35 – Emergency Loyalist Radiogram: This is in the same room as the LC Circuit C1 it’s in the left corner on the computer terminal.
29:42 – RNV Borascu Captain’s Log: This is in the Cargo Shipwreck side mission area. Kill all the enemies and the Flock that spawn. The Captain’s Log will be in the blue container that is open on the right side.
29:54 – UIR LC Circuit B1: In the same area as the Captain’s Log. Go up the stairs and it will be on the floor.

This next set of collectibles is quite important that you do it in this order I think. Reason being is that initially I completed the Cosmonaut Training Facility mission before completing the turntable mission via on chapter select. When I did this the bridge was shut and I couldn’t find a way to raise it. Meaning that I could not complete the Turntable mission. I think this is a bug, so just to be safe get the collectible from the Train Turntable area first before going to the Cosmonaut Training Facility.

30:08 – UIR Tag: Lieutenant Melich: This is in the Train Turntable main mission area. Kill all the Swarm and head on inside. When you are inside, do not interact with the console before collecting the UIR Tag which is in the far-left corner.
30:42 – Lightmass Missile Fragment: This is in the Cosmonaut Training Facility area just before you go inside. So you will see hole in the wall that you have to go through to get inside the building, go right of that hole and you will find it on the floor to the right.
31:07 – UIR Tag: Captain Tagger: This is inside the Cosmonaut Training Facility towards the end. Once you have killed all the enemies in the big circular room, head to where the Hammer of Dawn beacons are and it’s next to it on the floor.
31:30 – Fallen Nomad’s Visor: This is the very last mission in the Chapter. So you’ll want to take the Hammer of Dawn beacon you got from the Cosmonaut Training Facility back to the Airport. You will then get a call from Paduk saying his friends are in trouble. Head to the Nomad Convoy mission area and it will be on the floor on the right as you enter the area.

Act Four – Chapter 1
31:47 – Encyclopaedia of Tyran Military History: After Cole gives you a lift via the car. You will head down some stairs and it will be on the right on the floor behind a desk.
32:11 – Ruptured Gear Helmet: This is after you defeat the Swarmak and use the Hammer of Dawn for the first time. You will come to this road where there is a van in the middle of it. Bear right past the van and the collectible will be on the cliff edge.
32:33 – Cog Tags: Lt Mihn Young Kim: This is outside the Tombs of the Unknown and it’s on some rubble on the right side of the road next to a blue car.
32:49 – Restoration Notice: Tomb of the Unknowns: this is just before you go inside the Tomb of the Unknowns and it’s on the wall on your right just before you enter.
