Since no one has posted a guide to 100% yet, I figure I may as well try my hand at it. Please leave feedback in the comments!

The journey to 100% in GTAIV is long and tedious, and the reward is not worth it. However, the ratio and TA Score make it all ok in the end.

To get 100%, there are 7 main sections of things that must be done. To keep track of all this, a helpful checklist can be used, like the one located here: https://www.gta4.net/100-percent-completion-checklist/

Section One: Missions--60%
This part consists solely of all the story missions. There are 94 missions in total, but only 86 must be completed to get the 60%. These are the main story missions ("The Cousins Bellic" to "Out of Commission" (Revenge) or "A Revenger's Tragedy" (Deal)).

This is simple enough -- just complete all of the missions in order. If you mess up, use the cell-phone replays to quickly jump back in the action.

Section Two: Side Jobs--10%
This part is all the small mission-like duties available for Niko to do. Each set of missions is worth 2%.

Complete all 10 drug deliveries for Little Jacob.
This corresponds to the achievement

There are three main types of drug deliveries: timed delivery, stash delivery, and cop runs.
- Timed deliveries have a pretty tight time limit, I recommend getting in a faster vehicle than the beater Jacob gives you.
- Stash deliveries sometimes put you up against 10:1 fights, often with shotgunners in close range. Wear body armor for safety, as Drug Deliveries can't be repeated via cellphone.
- Cop runs are fairly simple, as you only recieve a 2-star wanted level.

Win all 9 races for Brucie Kibbutz at least once.
One would think this corresponds to

but it actually has different requirements.
There are three races per borough, and they are broken up into Dukes-Broker, Algonquin-Bohan, and Alderny. if you try to race in a borough you have completed all the races in, Brucie will tell you there is nothing going on. Complete all 10 Exotic Exports for Brucie.
This corresponds to the achievement

The vehicles are fairly simple to get, apart from a few missions where you instantly gain a 2-star wanted level when you take the vehicle. WARNING: If your email box is full when you deliver a car, you will NOT recieve the next email, rendering both the achievement and 100% impossible on the current save file. Complete all 9 missions of The Fixer's Assassinations.
This corresponds to the achievement

Complete all 30 car thefts for Stevie.
This corresponds to the achievement

I recommend not focusing on this, and just working on it as you go through the story/the rest of the 100% requirements. It takes a few in-game hours for Stevie to message you, and you might as well make use of it instead of waiting around. Alternatively, you can sleep a couple times to chug through these. Section Three: Friends--10%
This part is the stuff you do with your friends. It is important to note that for 100%, it is not necessary to interact with Roman or Dwayne AT ALL. Only Brucie, Little Jacob, and Packie matter for 100%. Each section is worth 5%.

Special Abilities:
All of these are the same. Get the 'Like' stat of the friend in question to 75%, and they will eventually call you and offer their ability to you.

Activities:
You must also go on every activity with every friend at least once. Not all friends have the same activities, so you only need to do the ones listed in their phone menu.

Brucie Kibbutz - Do each available activity once.
Brucie is available 07:00 - 01:00 (18 hours)
Brucie has 7 activities: Boating, Bowling, Drinking, Eating, Heli Ride, Show, and Strip Club.

Little Jacob - Do each available activity once.
Jacob is available 13:00 - 04:00 (15 hours)
Jacob has 6 activites: Darts, Drinking, Eating, Pool, Show, and Strip Club.

Patrick McReary - Do each available activity once.
Packie is available 16:00 - 06:00 (14 hours)
Packie has 6 activities: Bowling, Darts, Drinking, Pool, Show, and Strip Club.

Section Four: Games--5%
You must beat each game against an AI, whether that be a random person or a friend. These cross well with the need to do all activities with all friends, so try to beat them.

Beat a computer or friend at Pool.
This corresponds to the achievement

Michelle is the worst AI in the game at Pool, and Jacob is the best, with Packie also being quite good. Try to line up the shots carefully, and LOOK OUT for the 8 ball.

Beat a computer or friend at Darts.
Jacob is the worst AI in the game at Darts, and Packie is the best.
It is possible to win in two rounds, meaning even Packie is unable to win. In the first round, score 180 (also netting you

Beat a computer or friend in a FULL game of Bowling.
A half game will not count.
The best strategy for bowling is to take two steps to the left, and release the ball straight ahead at full speed. This will often result in either a strike, or one pin left standing. Either way, most AI are pretty bad at bowling except Brucie, who regularly scores 120-180.

Beat the high score in the QUB3D arcade game.
This corresponds to the achievement

Try to clear the blocks as fast as possible, while also going for the square clears, as they allow you to use power-ups, which are often VERY helpful.

Section Five: Random Characters--5%
Completing this will also net you

This part requires you to do all 16 random character missions. There are some requirements before some of the missions can be unlocked, mostly related to story progression. The stranger missions are:

Marnie (2 Missions)
Eddie Low (2 Missions)

Section 6: Police Work--5%
This part requires the use of a police vehicle, whether that is the cruiser, the patriot, or the FBI Buffalo. Each section is worth 2.5%

Complete all the Most Wanted missions via police computer.
This corresponds to the achievement
Grand Theft Auto IV (PC)ManhuntThe Manhunt achievement in Grand Theft Auto IV (PC) worth 63 pointsComplete the most wanted side missions from the police computer.

Each borough has 10 missions each, with Dukes-Broker-Bohan being lumped together. If you are too close to the location of the criminal whose file you view, the police computer will display a page reading "File Being Updated", and in most cases, your game will softlock. Be aware of the locations of criminals!

Do 20 vigilante missions via police computer using the "Recent Crimes" option.
This corresponds to the achievement

You can do any of the 'Current Crimes' in the police computer. I recommend doing all 20 in Bohan, as it is the smallest borough and it takes less than a minute for each mission. The fastest missions are the 'Criminal on Foot' so look for those.

Section 7: Collectibles--5%
This is arguably the most tedious section of 100% completion. The game does not autosave after each jump/pigeon, so make sure you save often to avoid losing progress if the game switches off. Each section is worth 2.5%

Kill all 200 Pigeons.
This corresponds to the achievement
Grand Theft Auto IV (PC)Endangered SpeciesThe Endangered Species achievement in Grand Theft Auto IV (PC) worth 258 pointsCollect every hidden package in the game.

Complete all 50 stunt jumps.
This corresponds to the achievement
Grand Theft Auto IV (PC)Dare DevilThe Dare Devil achievement in Grand Theft Auto IV (PC) worth 159 pointsComplete 100% of the unique stunt jumps.

Most jumps can be done in any relatively fast vehicle, but some require a bike to be able to correctly hit the landing area. In addition, most jumps require you to land on all wheels, and it is not possible to control a car's rotation in-flight. After you've done all of this, congratulations! The reward for 100% completion is unlimited ammo capacity, meaning you can buy as much as you want. Unfortunately, this feature is glitched, and if you save & reload the game when having more than the pre-100% maximum amount of ammo in a slot, it will be reset to that maximum.
