On the second floor of the building, speak to the tenant of room 206 and you'll get told that something shady is going on in room 205, which will start the case.
Enter room 205 and you'll see a very gross apartment with organs in jars among other things. To the left of the computer is a safe that you can scan. Stay in EM vision and you'll see wires heading to the left. Follow them to find an electrical switch that you can interact with to unlock the safe. In the safe, you'll find some disturbing photos that you can interact with that will update your case log. It says you found a code 7441 for a storage facility in the basement.
After going through the story and completing Helena's dream sequence, you'll access the basement. Go to room 028 and you'll find the organ farm and a computer linked to it. You can choose to keep it alive or kill it. To keep it alive, hit the green switch on the left and reboot the simulation. To kill it, you hit the switch on the right, pull the lever, and then take the plug out of the biomass.
The achievement should unlock after 30 - 60 seconds.
