  To swim with a horse you must first buy the ability from the stable in any town it cost 300 and afterwards just swim 3km collectively and the achievement will pop

  I used a combination of @InquisitorZac and @Halcyon Pure methode by ‘running’ with my horse trough shallow water along the river bed. And it unlocked pretty quick for me.

  I got this on the bobcat looking 'horse', using the sprinting method, took less than 10 minutes.

  I tried using the shallow water method. Started at the dock on the south-western corner of the north-eastern "island" in Suthsexe (a little east of the Cicestre Abbey) and ran counter-clockwise all the way around the perimeter of the island until I got back to the dock.
  
  The achievement didn't pop yet - and this after I'd played the game for 145+ hours and had to have gotten at least some incidental mount swimming - so I started doing deeper water swimming around the perimeter again, while keeping the horse in "sprint" mode.
  
  The achievement popped after about 5 minutes of that. This is on Series X, using a horse mount.

  First purchase the ability to swim for 300, then put a waypoint on anything close to a river that's further than 3km, and swim to it while on horseback.

  Good idea with the waypoint. I did 3 lengths of 1000m and unlocked it within 10 mins ish

  If you are planning on playing through the entire game, you're looking at - realistically - 200 or so hours of gameplay. If you bite off a little at a time throughout your gameplay, this achievement will eventually pop for you.
  
  Once you have a stable, buy the swimming ability, and then any time you need to cross a river or body of water, do so on horseback. Those little bits travelled eventually add up.
  
  I accomplished this task after about 150 hours of gameplay. I had not yet started Francia, and I just finished up Eurvicscire questlines.
