  When you are near an enemy, 2 prompts will come up: kill or use as human shield. Hold to grab the enemy. Do this 4 more times and the achievement will pop.

  Very Simple, on the first mission, as soon as you are heading through a house and out a window (story related), it will prompt you how to perform a takedown or body shield, perform the body shield, die and repeat a further 4 times.
