First you need to beat the game on Killer difficulty so you can play the unlocked chapters on that difficulty.

In order to select Wesley without the mask as the playable character you need to unlock him by going to Secret Codes and entering 0100 0011. (When you play through the game regularly Wesley is wearing a mask in this chapter).

Then select Chapter 7 in Unlocked Acts. The battle with Arana is about 2/3rds of the way through the chapter. Once you beat her (which is pretty easy even on Killer difficulty) the achievement will unlock. You don't even need to finish the chapter.
