For this achievement you need to either upgrade a green or blue rarity item. It is also worth noting you need to reach a certain point in the story before you unlock the option to craft.
I recommend doing strictly green items as blue items require titanium to upgrade which are very scarce whereas green items require iron for weapons and leather for clothing which you can get from dismantling and harvesting ores (iron).
I saved up a bunch of green items and iron and leather and went and simply improved the rarity of 15 items, it cost me around 300-500 iron for each weapon and the same in leather for clothing items.
    Happy hunting!

Gloss666Thanks for this guide, didnt realise greens only took leather or iron to upgrade, so makes this cheap as chips towards the end of the game

AaruniThis was a huge help I honestly thought only upgrading rares counted

TymanTheLongTo get this achievement definitely upgrade greens (you can lower your world tier difficulty if you need to loot more or play on a low level character). You can loot all area drops (save chests if they’ve not been opened) by hitting down on your D pad.

However, if you know what stats you need be on the lookout for Blues with perfect stats, upgrading to epics (purples), will add an extra tier 2 upgrade and if it’s the right one for your build you can replace the tier 1 mod with a second, ideal mod, it’s a great way to get the gear you need

    Fastest, easiest and cheapest way
    1. Only improve the green items
    2. You will have a lot blue and purple items while you play the game
    3. Put some blue items in your inventory box
    4. Create a new character, get the blue items from box and sell them, buy the greens and put them back into the box
    5. Use your old character to improve the greens
