For this achievement, press the right bumper and select "Combat Arts." At the bottom of the menu that just appeared are four symbols under the category "Combinations." Simply select an empty symbol/slot, add which ever combat arts you want, then press the Y button to add that combination to your hotkeys.
Use your homebrewed combo on a baddy and this achievement is yours.
