  This is the second easiest out of six "Complete 5 missions with X squad member" achievements as it's related to your missions with Ashley Williams, who is the second companion to join your team. Simply keep her in the party after Eden Prime and some early recruitments on the Citadel, and the achievement will pop-up quickly.
  
  Of course, if you want you don't have to do it immediately as the achievement doesn't need consecutive missions. Just 5 total in the whole game, but if you are new to the series, it's advised to them before the Virmire story point.

  Does anyone know if you the squad member needs to be in the group when the mission was collected or do they just need to be there at the point when the mission is completed?

  I've done more than 5 missions with Ashley but it still hasn't unlocked. Anyone else having this issue?

  @T1OOO - I'm having the same issue. I've completed five assignments and two main quests with Ashley in my squad and it hasn't unlocked. I'm switching between my Series S and my Series X.

  Update: For those not getting the achievement to unlock, I hard reset my console (held down power button for 5 seconds), unplugged the power, and then rebooted the game. It popped as soon as I loaded in to my game. Hope that helps!

  Still don't have this achievement, i've done everything suggested, even KamalaCackle good idea; No Go. Hopefully, I might be able to get it in New Game+...

  I have a theory... Maybe the achievements will not unlock if the group split up BEFORE you give back the assignments on the galaxy map. It happens a lot when you do the assignments that you give back quests alone inside the Normandy... So maybe the game didnt consider your allies did the quest.
  I was with Ashley and Liara achievements missing, i did two quests in the Citadel and both achievements popped, after i did a lot of assignments with them both and no cheevos... So maybe that's the problem.

  I thought I was having trouble with this, I went down to planet after planet with the soldier and nothing unlocked. However when I was running around the citadel completing some of the side quests this actually popped. It looks as though the quest/side quest needs to be completed with the member by your side. Hope this helps :)
