This is culmulative. Every incursion have 3 objectives, one in each of the 3 sub-zones, that you move in-between using the airlocks. So it takes a minimum of 15 standard incursions for you to unlock this.
You can't go DBNO ("down but not out") in the entire match for this to count. This means reaching the third sub-zone and completing the final objective without being revived once.

This achievement will probably come naturally as you complete the rest of the game, specially if playing carefully and using stealth as much as possible. If you think this solution is incomplete, please feel free to leave feedback and i'll promptly update it.

    A few tips:

    • For this achievement you don't need to complete the first two objectives. If you're in a hurry you can rush to the first two sub-zones airlocks and go directly to the third. Don't really recommend grinding it this way as you will probably get progress naturally as you complete the rest of the game.
    • I got this playing solo. The game scales difficulty and enemy quantity depending on the amount of players. Once you get in a good rhythm completing incursions alone is not that hard at all, even on higher difficulties.
    • Highly recommend to always have at least one operator in the squad bring a drone. Best way to scout ahead and plan your approach. It makes stealth much more viable.
    • Try to pick operators and equipment according to the objectives. Defending areas or explosives are best suited for operators like Gridlock or Jager, using claymores/frags. On the other hand objectives that requires you to be stealthy, like saving a hostage, would be easier with Vigil or Pulse and equipments like paralysis grenades/flashbangs.
    • Enemies and objectives you get in a match will always be random. If you don't like the objectives you were assigned, you can extract as soon as the incursion starts and choose to replay. The game will randomize new objectives. Useful when you get to know which ones you're more comfortable completing alone.
    • Conserve as much health as possible. Everything in the game will kill you ridiculously fast even on the lowest difficulty. Search for medkits to keep you boosted and recon the area ahead before rushing in. If you're unsure you can go through an area unscatted, find a different route if possible. Don't pick unnecessary battles unless you're confident your health bar won't be drained.
    • There's no shame in retreating. If you're discovered and there's too many enemies to handle, just run for your life. Try to go back through cleared paths, get as much distance as possible and close doors behind you. The enemies will eventually calm down and stop searching.
