The walkthrough made by Johnny Sinister will guide you to each item. Here is the full list and location if you need a quick and dirty checklist to ensure you've gotten everything (like I did).

At this time the team has supposedly fixed the bug related to this achievement (counter only increments when opening chests that contain equipment), but beware nonetheless and plan accordingly. The list below recommends saving the chest in the basement of the dorm until after you've obtained the last story related equipment.

UPDATE 4/28/2022: Please also note that there is seemingly a bug with the ORANG-ISH GLOVES item relating to this item not entering your inventory correctly or at all, so make a save before buying this item. A note regarding this is now listed under Ep. 3 below. Please read the note below for further details.

Please also note that the "Snow Level" item may also be unobtainable after that episode as reported by some users.

I just got this achievement, following this guide. Thanks to the author and every who contributed to it.
What I did is to just pay attention to this 'orangeish gloves' warning note. I have saved before buying it, and saved again right after I checked in my inventory everything displayed correctly. Icon, name and description were just fine, so I could continue.

On 'day 7', I opened the chained chest that was marked as preferably being opened as the last one (in the Dome area), I didn't see I had to save it for later, so I was a bit (A LOT!) afraid of the bug, I didn't want to restart a fresh new save!

But right after I won the finale in the tournament, I got the last equipment item AND the achievement. I was close to restart from scratch...

Yeh, I got the glitch, even getting the golden in the last day....
I search and I saw how the fix it in steam and tell to buy something to fix the Cheevo.
So I tried and don't.
Even I equip the comfy to someone and the orange ish to another character, that fixed the names in the list but no Cheevo.
And yes, I checked all the items in the list.

Nice list and clear warning. The formatting could definitely be improved to easily distinguish items how the get it. Additionally, highlight the missable ones ie side quests

found out i was missing brand new bandaid (literally to the right of the nurse, not in the room on the right because that chest has the first aid kit i think).

Tips: Save at least once per day manually so you can go back in case you miss something etc

all the gear was in the WT but in a different order than your solution. Using your solution I cross referenced my equipment and found I missed "Secret Note". I had the weird glitch with the orangish gloves but it didnt prevent the achievement. I panicked tho and thought it did. Ty for the solution.
