This achievement is tied to the boss and the end of the water cave in Ganath Haros. The boss has two phases. The first one is a big slime boss that keeps calling smaller ones for help. They have poison and are weak to fire. The second phase is easier, the boss is huge and keeps using water magic. Just get behind him and keep using the Blazing Sword until it gets downed.
