    Here is my 100% Achievement guide.

    The main and best advice i can give is constantly hit the cn_LB & cn_RB buttons simultaneously.
    That way you never die, as long as you keep dodging, you never die.

    As soon as you start the game, simply hold the cn_RB button until you reload.

    This should only take you around 5 minutes or so to complete :)
