  You need to pick up 2 extra lives and have 5 lives in total
  At the beginning of the 3rd level, kill the 2nd and 3rd enemy over and over again
  The 3rd enemy usually drops an extra life after a while

  ^ It took me 2 minutes to get 2 lives, you must be doing something wrong

  @pongulf92 same, shooting the two enemies repeatedly and getting only health from time to time, never the "head icon". 10 minutes and counting

  It eventually took me close to 30 minutes to get the two head icons. Your mileage will definitely vary

  Took me about 10 minutes of farming then 2 lives dropped back to back.

  Nearly 15 min of nothing but health pickups and then the 2 lives dropped within about 1 minute of each other
