  Casey Jones is unlocked after completing Story Mode on any difficulty. Once he was available I started up Arcade Mode, selected him, and the achievement unlocked as soon as the first level began.

  You unlock Casey Jones after you complete the story (on any difficutly) for the first time. After you unlock the char, go back into the game, select Casey Jones and the achievement unlocks in the episode.
