How to unlock the Pacifist achievement

  You'll need Double Jump which can be purchased for 20 ore after unlocking the Super Shotgun and Plasma Rifle.
  
  Load up the first level "Sacrum", when you pass by the room that ends the level you'll walk into an open area with an automap collectible, double jump to the ledge beyond the automap and fall down onto the bridge below, this skips all the forced encounters in the level.
  
  It didn't unlock for me on Ultra Easy but it turns out a single kill rounds down to 0% kills so if the achievement didn't unlock something would have died to friendly fire. Thanks to Aboge and Q u 4 lK e for helping to clarify.

  Possible to combine Hard Target with this, confirmed. Ultra Easy as well.

  On Sanctum there is an enemy that ALWAYS explodes as you take the end elevator down invalidating the run for me. There is a barrel, and the enemy runs right into, killing himself on purpose.
  EDIT: I knocked it up one difficulty level, and he still killed himself, but I got the achievement... so maybe try very easy instead of ultra easy

  Here's a clip illustrating D4rkTrooper90's method for the 'Sacrum' level; can be done in under 2 minutes. I actually managed to avoid taking damage in this run, so I would have also unlocked the “Hard Target” achievement too had I not already done that.
