How to unlock the Hellfire achievement

Evolving weapons happen by combining a max level weapon with any level of a specific secondary support item. Once you have both of those items you must then kill an Elite enemy and open the chest that it drops. The combinations for the Fire Wand is the following

Hellfire – Fire Wand + Spinach

Fire Wand is unlocked for all characters after destroying 20 Torches and Spinach is available by default.

Thank you. What is the Max Level of a weapon? 10?

The default Max levels for weapons are usually 8 but I believe a handful can reach up to 9, you can view the limit and current level of your equipped items in the pause menu on the top left! :)

I've evolved the fire wand into hellfire at least 6x now and this achievement still won't pop. I've gotten most of the other weapon evolutions.
