  Best time to get this done is after the CCTV sequence and gain access to the prison. You'll then rescue Alejandro and have the opportunity to refill your back pack with flashbangs, molotov, semtex, and frag grenades. After taking care of the riot shield guys you'll immediately free prisoners, regroup, and drop down into an open area where the Shadow Group will be expecting you. There are double-doors on your left in the back and towards the right where you can easily flash/gas the enemy and finish them off with a molotov, semtex, and frag.
  
  If you happen to get unlucky, you can always reload the last checkpoint and try again with a different combo until the achievement pops. This is also a good opportunity to get 3 riot shield kills as well.

  Til this day i had trouble getting this, but i can finally confirm using the tear gas grenades is a must. Flash bangs not working.

  After reloading flash and grenade , i kil 2 Shields and 1 shadoow
  And nothing pop

  Remember, you must kill with all 3 lethal options. I know this may seem obvious but you would be surprised.

  Actions

  Done on Mission 14: Prison Break. After the CCTV section with Ghost you'll then open a locked door. This is your first opportunity to get enemy kills with a full backpack of equipment. Ensure you throw a Tear Gas and the hostiles are under its effects before killing them with a Molotov, Semtex and Frag grenade. If the Tear Gas starts to wear off throw another one down (you have 4 of everything).
  
  If you're unsuccessful just "Restart Checkpoint" and try again. The checkpoint is just as you enter the corridor too!

  Super easy based on this guide! Just like the guide mentions one with each equipment, the tear gas works best for effecting the enemies (you’ll know if they’re effected by tear gas when they cough) and just go at it!

  Best guide thus far. I tried following the other ones and had no luck. There are no armored enemies here and it is close combat in a small room. Makes it the easiest way to get it imo.

  The 15th mission of COD MW2 'Prison Break' features numerous opportunities to grab this achievement.
  
  While the achievement is self explanatory, there have been issues attempting this achievement before rescuing Alejandro and throughout the level.
  
  I recommend saving the achievement until you enter the cafeteria/break room.
  
  From there, there will be enemies to the back left door, stun them and throw the semtex.
  
  Look to your right to the armored enemy. Stun him (and not yourself as I did in the video) and hit him with the molotov.
  
  The last guy in the way back left corner should be behind a cart. Simply stun/gas him and throw the frag grenade.
  
  The achievement should then pop. If it doesn't, reload from last checkpoint and attempt it again!
