Feathers can be found in all cities, on rooftops, chimneys, windowsills, and overhangs.
They will glow white. If you turn on Eagle Vision, they will become even more visible, as the glow stands out more.

You can see your progress by checking the DNA stream in the menu. Hit A once over the Feathers progress to see individual city progress.
You can also narrow it down to areas of the city.

You must put the Feathers in the chest in Ezio's mother's room in the Villa.
At 50 Feathers, you get the chance to buy the Condottiero hammer for 30k Florins.
At 100 Feathers, you will be given the Auditore cape.

Here are is a guide to help locating each feather:

You do not have to get all feathers before the end of the game, as after the end you re-enter the Animus and are able to run around and collect Feathers.

This is another good guide. It has maps showing all feather and glyph locations. You can click on the location and view a vid on how to reach the feather/glyph. Also, what is really nice about it is they have check boxes so you can check each one off as you find them and know EXACTLY which ones you still need.

This is a quick note, if you wait to collect the feathers in Florence till after you beat the game, then you no longer can take advantage of Medici cape, since Lorenzo is no longer around. So I would maybe recommend getting the feathers in Florence before the time skip to Memory 14. [I have been informed that apparently the cape still works, not sure to tell you truth if it was just a glitch, but its not like it matters]

Tuscany is by far in my opinion the hardest place to get all the feathers despite only having 11, the second least feathers. In the city itself the majority of the feathers are on beams sticking out of look out points, the majority of which will require you to climb at least part of the tower. So just a heads up, if you are using a guide and it says a feather is in an area and you cannot see it even with eagle vision, chances are its on one of the towers.

And to reiteriate what another users pointed out, under secondary memory you can see exactly how many feathers you have period, how many you have in a respective city, and how many you have in each area. As a result in a worst case scenario you will never have more than the 12 feathers in Flori to keep track of. In other words you will not end up with the big problem in Assassin's Creed one, where you have 99/100 feathers but have no idea where the last one is.

Harlequin God@jonas did you check under secondary memory in the settings, it should tell you where you are missing your feather and you go from there.

Mr GranstaffWhen does the time skip happen exactly? Is it right when you complete Memory 11 (immediately after, in other words) or do you have to trigger a Memory Start after Memory 11 completion?
