    The Queen appears at the end of the Bird & Elephant level once you've completed the level previously AND rolled up the King of all Cosmos. See that solution for additional information:

    Like the King, she is REALLY big. You'll need to be about 3000m to be able to roll her up.

    Here is a video of my run through the level in As Large as Possible mode where I rolled them both up:
    A few notes:
    1) Per Jamone the Afro in a comment on the King solution, this can be done in Eternal Mode which is unlocked after completing the level in As Large as Possible and As Fast as Possible.
    2) If you bounce the Queen because you aren't large enough to roll her up directly, she will wake up and run away after several seconds so be prepared to roll her up ASAP.

    vSullyI’m not sure what caused your problem, but any item that you can bounce, you can then roll up. You can’t bounce something that’s too big to be rolled up.

    GoatlipsIt's at 16:30 where the king gets rolled up, at 3214 M with only 90 seconds left, and the queen's seconds later (but it's hard to spot):

    ...I saw the king on that level, thought he was trying to help me, but the most I had was 1760 M, so couldn't have grabbed him anyway, so hopefully Bird & Elephant Eternal works (Confirmed - I got the king rolled up on Eternal and got the achi when I quit it, then I did Eternal again for the queen achi)! Luckily we don't have to roll up 1 million things (roses) for the rose (which would take about 48hrs!)!

    The easy mode is picking up de king in eternal mode one time and finish.(elephant & bird) the first time I played the Queen wasn't there.

    Then I made a second run, and yes the Queen was in the game! Thats it, reach 3000m and the cheevo is yours.

    Edit: found the answer. You can quit by pushing X at any time.
