This achievement requires you to find all 16 records in the game. Some are only available in NG+ while others can only be received by responding with specific responses, making them missable.
I’ll list all the records in order of when you can encounter them.

1.
Feel (Missable) In Krat City Hall you’ll come across a woman in a window asking to retrieve her baby. Say you’ll do it and keep progressing forward to find the broken baby puppet. Once you pick the puppet up head back to her and make sure you say
She’s a cute baby to get this record.
2.
Divine Service (Missable) In St Frangelico Cathedral Library you’ll come across an NPC named Cecille. She will ask you to retrieve the Archbishop mark. Proceed further into the level to find the mark then head back and give it to her. Make sure to give Cecille the mark BEFORE you kill the archbishop or you’ll miss this one. After you’ve given her the mark go kill the boss of this area and return to find this record where Cecille was standing. Do this ASAP as Cecille will become infected if you wait too long.

Someday In Malum District, once you reach the Red Lobster Inn look for a ladder that takes you up to a merchant. This merchant will be selling this record for 3,800 ergo.
4.
Fascination (Missable) This record can be found in the Estella Opera House in Adelina’s room after the Puppet King fight. You MUST give Adelina the red apple that Polendina sells to her before you kill the Puppet King or you’ll miss this one. Polendina sells the red apple once you give him the Krat supply box that can be found in the St Frangelico Cathedral Chapel. Once you kill the boss return to her room to find this record.
5.
Why (Missable) This record is super missable. First off, once you’re in the Grand Exhibition area you’ll need to find the NPC named Belle. When you’re up top walking across narrow beams you’ll see a ladder leading down to where she’s at. Exhaust all her dialog and then proceed through story.
Be sure to speak to her before killing Champion Victor or this will become missable. She will return to Hotel Krat and give you a mission later on. Once Antonia tells you about the secret passage go and speak with Belle. She will want you to go look for her partner which can be found in Krat Central Station Platform. Her partner can be found in a train cart underneath and behind a staircase (right before the Robber Weasel fight). Her partner looks like a normal zombie enemy so make sure to NOT kill him or it’ll mess up the mission. After speaking with her partner return to Belle and say,
He was killed in action fighting a puppet. You must lie to get this record. She will give you this record afterwards.
6.
Quixotic (Missable) Near the end of the Grand Exhibition area after the boss fight, you’ll meet up with the Red Fox and Black Cat. Give them a gold coin to receive this record.
7.
Memory of Beach (Missable) Later in the game Polendina will tell you that Antonia doesn’t have much longer to live. He will ask you to help with a cure. I believe after killing Champion Victor, go and speak with Giangio near the tree and give him a gold coin in exchange for a cure. Afterwards go and give the cure to Polendina. This must all be done before Hotel Krat gets attacked. As you progress further through the story you’ll learn that Antonia has passed away (after you defeat Laxasia The Complete). Go upstairs to Geppetto’s room to find this record in her wheelchair.
8.
Misty E’rA (Missable) This one can be found in Hermits Cave. To access this area you must first get the rustic cryptic vessel. This vessel can be found at the Collapsed Street stargazer later in the game. Look for a chest requiring you to jump down from a ledge to find the special krat supply box. Now head back to Hotel Krat and give the box to Polendina who will now be selling the vessel. Buy it and decipher it from Vegnini and then head to the Barren Swamp Bridge stargazer. Turn around and you’ll notice the gate to this area is now open. Progress to the end of this area and kill the elite to receive this record. Make sure to do this before the end of the game since Vegnini will disappear making this one missable.
For records 9 & 10, you only can get one or the other in a single playthrough. You’ll have to go for the one you don’t initially get in NG+-Also make to sure to give Alidoro Eugenie’s gloves when he is outside the Barren Swamp stargazer to be able to kill him at Relic of Trismegistus.
9.
Proposal, Flower, Wolf, Part 1 (Missable) For this record you must kill Alidoro at Relic of Trismegistus. Take his vessel over to Vegnini and be sure to read through every page of the deciphered vessel BEFORE speaking with Eugenie. If you speak with her first then you’ll miss out on this one. Once speaking with her say that he was her older brother. After speaking with her go rest at the stargazer and then go speak with her again to receive the record.
10.
Far East Princess (Missable) This record is similar to the one above in terms of how you get it. Pretty much do the same thing as above, kill Alidoro, head to Vegnini to decipher it, you don’t have to read it this time. Talk with Eugenie and tell her that Alidoro was just a talented stalker for this record.
11.
Shadow Flower (Missable) At the Ascension Bridge, near the end of the game when you meet Sophia choose to give her peace for this record. You MUST choose give her peace to receive this record.
NEW GAME+ RECORDS12.
Proposal, Flower, Wolf Part 1 (Golden) (Missable) This record can be bought from the merchant near Cerasani Alley, right before the first boss fight for 6,000 ergo. Make sure to buy this one early as this merchant will disappear later throughout the game.
13.
Quixotic (Golden) This record can be purchased from the merchant at the Inside the House at Elysion Boulevard stargazer. This record costs 10,000 ergo.
14.
Shadow Flower (Golden) In the Lorenzini Arcade area you’ll come across a merchant near a fountain where you get the key in order to progress. He sells the record for 10,000 ergo.
15.
Memory of Beach (Golden) This record can be bought from the merchant at the Abandoned Apartment stargazer near the end of the game. The record can be purchased for 10,000 ergo.
16.
Fascination (Golden) In the final area of the game you’ll come across Test Subject 826. At the Arche Abbey Outer Wall, near the final trinity room look for a passageway guarded by a boxing elite and take the elevator down to find him. Exhaust all dialog and then proceed to fight Laxasia The Complete. Once you kill her, fast travel to Rosa Isabelle Street. Look for the bridge overlooking the opera house with fire in the background to find Test Subject 826. He sells the final record that you need!
Once you have all the records make sure to listen to them all to get the achievement. You don’t have to listen to them fully, just let each one play for a couple seconds then switch to the next one.
