You must play through each characters story on hard, not just one character. You do not have to start a new game for this achievement. It is highly recommended that you complete the game first on a lower difficulty to collect skills and level up each character to level 10 then change the difficulty to hard and replay each mission.

You will want to skip all the side objectives and just try to complete each level as fast as possible. You do not want to spend half an hour clearing an entire level only to find out you can't defeat the boss and instead want to try maxing out some different skills and using them.

Radiant Sphere with Magic Empower both at level 5 are extremely effective on many bosses but not all. Prism Ray is a good alternative to try when Radiant Sphere is not effective. Lightning Bolt and The Void are great skills for dealing with large swarms of enemies.

When making heavy use of skills you should use a passive ability that speeds up their cooldowns such as Ethereal or Quick Melee/Quick Magic. Quick Draw can sometimes make skills recharge even faster but only if you use it in combination with another passive ability, do not use it in place of Ethereal or one of the other Quick abilities.

The 2 skill elemental combos for ice, lightning, and gravity are extremely powerful when maxed and can kill large hordes and summoning towers (just ice and lightning) very quickly.

Vitality at level 5 may be a big help for some boss fights if you are really close but not able to defeat it. So level it up if you have the experience to spare. That way you can swap it out on the game over screen without having to restart the level just to level it up.

Endure Fire level 5 makes you immune to fire damage but not explosions. With this on during certain levels you can just ignore the fire environmental hazards. You can die later in the level and swap the ability out for a different one if you no longer need it for the rest of a level. Endure Curse can be very useful on some boss fights or other harder sections if you keep getting silenced and unable to use your skills. Do not use the Endure Elemental passive because it does not offer as much protection as the individual ones.

Finally the most important thing to remember especially if you are going for a caster build using skills and not X/Y mashing melee. KEEP MOVING. Don't give enemies a chance to swing at you just keep moving and be ready to evade when needed. This is the only way you will ever have a chance of defeating certain bosses.
