Continued for 30 times (cumulative).

Just as the achievement description says, but it seems that continues in certain modes do not count. I'm sure that Arrange mode doesn't count.

Modes able to unlock this (confirmed): Xbox 360
Modes able to unlock this (supposed): Xbox Novice, Arcade
Modes unable to unlock this (supposed): Black Novice
Modes unable to unlock this (confirmed): Arrange
* Changing setting in OPTION -> Game Config disables all achievements.
* Using Restart from pause menu disables all achievements in that play.
