How to unlock the Adopt Or Die achievement

As littlejay2o02 said, you need the Family Pack from the Road to Rule to be able to adopt a child.
What you don't have to be though is king/queen. You don't need to be anyone "special", as soon as you get Bowerstone Industrial, you can reach the Orpahange. But, as the others said, if you become king/queen and chose brothel instead of Orphanage, your chances to adopt will be lower.
The easiest way to reach the Orpahange is to fast travel to Bowerstone Industrial. When you get there, you see a bridge before you. Go straight ahead till you see a second bridge, not far from the one mentioned. Go through the bridge (to the left side) and you will see some beggars sitting under a fence. That fence is the Orpahange's fence. The building is distinctive, because there is a rather large (and dark) yard in front of it and its door is unusually big.
A red-headed woman (named Linda if I recall correctly?, quest called Kidnapped) with some folks is standing there with a 30 seal quest waiting for you, if you haven't done it before. If it's available, then it could be a useful fast-travel right from the map - easy Orphanage, tada!

When you're inside, go straight ahead, don't bother with the first doors. At the end of the hall lies the room you need - it's on the right with children's beds. Pick your favorite child and adopt him/her. You can adopt as many as you like, after the procedure, you have to select a home for them and a nanny will be instantly hired to take care of the kids. You can also have multiple children in one house (my selected houses only allowed two/house though).

Sophiasoit's the last room on the right ("at the end of the hall"), the first one is not important :)

deathdude5just like to add a something to the guide... i managed to get just about the entire kingdom killed by letting the max amount of people die at the end.. and there are literally no children to adopt as the orphanage is empty

You need the Family Pack found in a chest in the Road to Rule for this achievement.

Go to Bowerstone Industrial. Go inside the Orphanage at the side of town opposite the water front (far back side of the industrial area). It's the building with the tall bushes around it.

Once inside, go down the hallway and turn to the right. There will be several beds with orphans standing in front of the beds. Walk up to one, and press A to interact with them and you will have the option to Adopt. Once you press A, the achievement unlocks.

If you turn the orphanage into a brothel later, the orphans will be on the street to adopt.

You do not have to be married.

GuruThaGreatI didn't know if I could adopt after turning the orphanage into a brothel, since I didn't know to look on the streets. I was worried I was going to have to start a new game just to get this achievement. Good thing I saw this solution first lol.

    Thanks.

"NOTE: If you do not help the orphanage when you are a ruler and turn it into a brothel, you cannot get this achievement unless you start a new game."

This is Wrong. If you do that, the kids are on the street, sleeping in cardbox. This is not a misseble achiev! ;)

BrutalSleeperIf you wait after the population is killed and just play through a few side quests and stuff like that eventually the population does rebuild.

oR33DoI see the kids in the street, it won't allow me to interact with some, some they will. However, I can't seem to adopt one. I did turn the orphanage into a brothel however.

Dave SimonI needed the money, so grudgingly turned it into the brothel but managed to save everyone by buying tons of property and raking in about 10 million gold. Good to know that I'll be able to get the achievement though. I saw the kids there buy didn't know you could still adopt.

On second play-through I intend on being thoroughly evil, Mwahahaha.
