You can only get this on your Encore (2nd) playthrough.

First, you need to defeat Rondo at the top of Aria Temple, to get the Heart Pendant. After defeating her, there will be a few cutscenes, and chapter 6 will start. You need to first go investigate both Prince Crescendo's room (2nd floor East of Baroque Castle) and then Princess Serenade's room (2nd floor West of Baroque Castle). After finding that neither are there, you'll get the Warp Room Key from someone who comes up and meets you after you exit Princess Serenade's room.

Make sure that you have not given the Heart Pendant to anyone to hold. It must be in your inventory without being an attached item for any character.

Now, in the Warp Room, use the LEFT warp to warp to Ritardando Beach. Go in the cave to the left, and at the fork, take a left until you see someone named Former Developer.

Talk to them until they repeat themselves, and then go back to the warp point that you arrived on the beach with. Go back to the Warp Room, and now leave the warp room. Go down the stairs in front of Baroque castle, and go along the right alleyway. You'll see an old woman, named Former Servant, half way along. Talk to her until she has nothing else to say.

Go back to the Warp Room, and warp to Forte City. Leave Forte City by going south, and go all the way through Hanon Hills to get back to Fort Fermata.

The following directions are assuming that you have not gone back to Fort Fermata after leaving it earlier in your second playthrough, and assuming that you have not moved the warp room.

Go up the ladder to the right of the room that the boss of Fort Fermata was in. Leave the room. Go north and take the only pathway you can that leads to the right. Follow it until you see a ladder on the left/inside, and go down it. Go north along the ground floor, and you'll see another ladder. Go up it, and go south. Follow it south, and then when you can go south no more, go left, and you'll be able to cross the moveable room to get onto the straight, middle, vertical part of the room. Go north and then cross through to the West Side of Fort Fermata.

On the West Side, go north to the outer ring of Fort Fermata. Follow it anti-clockwise until you find a ladder on the right/inside, and go down it. Go north, passing a couple of enemies to go to the locked door where Chord is hidden. Open the door with the key you will have gained earlier in the game, and then give Chord (the creature in the back right corner of the room) the Heart Pendant. After a bit of animation, Chord will disappear, Allegretto will pick up a Pot left behind by Chord, and the achievement will pop after you regain control of the characters.
