This comes from beating *every* campaign level on difficulty level 4 or higher.

That is, you not only have to beat the 33 "regular" levels but also the 40+ "challenge" levels as well at difficulty 4 or higher. It takes a bit.

Difficulty 4 is 4 stars, 1 below the hardest.

You can do the levels in any order, personally I did the 33 regular missions 1st then mopped up challenges in more or less random order. After beating the last one and getting back to the level select menu the achievement popped.

As for tips, personally, aside from Xaero (the final boss) most levels are pretty straightforward. For the most part I just tried to keep bots away from armour + health, picked up as much as I could for myself, and used the rocket launcher like mad (don't forget about splash damage, try leading bots down halls and fire ahead of them). For non free for all or team DM matches, I'd focus on the objectives pretty much ignoring kills.

To give an idea for those trying to get a feel for the difficulty (this is of course subjective): all-in-all I only failed two matches in the entire time I played the game -- the final boss battle against Xaero, and one random match in-between. Compared to say a CoD on Veteran, Gears of War on insane, or Halo on legendary this isn't bad.

Also note that frags in SP count towards the 1000 frags achievement. I unlocked that one long before I got Hardcore. (if you think about it, there's 33 regular levels, most of which are 20 frag matches, so that's 600+ frags there, so if you average 10 frags per challenge youi'll have the 1000 long before you finish them all).

Edit: in the comments Marx0r reports a possible way to make this even easier (note that I have not confirmed this, and others are complaining that it does not work, I'm just passing it along):

"If you load up the game, then start an arena on 1-star difficulty, immediately back out, then start it again on 4-star, the bots remain at 1-star difficulty. This effect remains for some time, you can beat that arena and start up another and the bots will still be easy."

So if you find yourself stuck, maybe give this a try.
