For this achievement you need to kill the boss of the Medical Pavilion known as Dr. Steinman. Steinman is a leadhead splicer with increased health and a machine gun, and can be identified by wearing a red surgeon's uniform.
When you see Steinman send his security bot after you and activate a turret, try to shock both with Electro bolt and hack them. If you replaced Electro bolt then destroy the security bot, and either leave the turret to be hacked later or destroy it now also.
After that there is a health machine before entering the surgery area, you should hack or destroy this (it can be bought out for $10). After Steinman shoots out the observation glass, head in and find where there's a water-logged area, and either hack or destroy this heath machine too (it can be bought out for $28).
Now since he can't heal, start shooting him with machine gun rounds (anti-personal if you have them) and shotgun shells (electric buck if you have them). If he goes into the water try shocking him too. If you keep doing this he should die very quickly and the achievement will be yours.
