For those of you unsure where to find this info, go to My Completion Times
Sadly this is not accurate if you got offline achievements prior to joining TA. I know I started and finished Fable 2 in 2009 but I didn't join TA until June 2013 which is the date the offline achievements were registered - giving me about a 4 year completion time for a game that took me less than 1 year.

For me Fallout 3 holds the record at 1334 days (around 3 years, 8 months)

Technically the game that took me longest to complete was Bully, however it was the original version on the PS2 I started back in 2006 or 2007 and never finished before I got my Xbox. I picked up the 360 edition this year and finished that recently - giving me a completion time of 8 - 9 years. But that's across multiple platforms so I doubt it counts.
Seen this thread had been recently revived (can't believe it was originally posted over 4 years ago), but I've now got a 10+ year completion. Dead Space. Started December 2008, finished October 2019. That's 10 years and 10 months.
Stoke me a clipper, I'll be back for Christmas... Whatever!

Longest in terms of how long it went without being completed is Crackdown at 4480 days. Longest just in terms of hours it took me to complete the game was Monster Hunter: World (base game). Admittedly, I could have cut the time way down if I went only for the gold crowns rather than messing around and grinding for the weapons and armor I wanted, but it took me about 430 hours to get the first 1,000 points.
