Ok, we will start off with the basic 'in game' achievements and story mode. I did these in kind of the same order (except I got all the basic 'in game' achievements BEFORE doing story mode), but this way should save you more time. First for story mode, you'll create your character. Just set what skills you want upgraded and once you've done that you'll unlock...

Now that that is out of the way, you are ready for game play. Story mode is set up to where you have to win games to obtain 6 players to add to your team upon victory. You can choose any path you want (which the game really only gives you 2 I believe), depending on what characters you want on your team. I would suggest going to Pablo's Sandlot first since Pablo is super fast and can easily net you in game achievements. Also, once you get to the Webber twins, whichever one you choose to get, will end up being a great WR.
Now when setting up these games, set them to Easy and 1 minute quarters. That way you save time and you wont be spending forever getting through Story mode. You'll be able to do a lot in these 4 minute games so don't worry. From Game 1, focus on Sacking the QB as much as possible in the game, as well as running the ball a lot too if possible. Here's what I was able to knock out in my first game. I'm going to split these achievements up so you can focus on certain things to get the most out of your story mode games. Focus on a shutout as well and go with Pablo's Sandlot first.
Game 1

Plus or minus a few others (like if you pass the ball any in this game) you should easily be able to get all of the above by focusing on running and sacking the QB all game. A lot of times when you sack the QB and dive at him, you'll cause him to fumble. Fumbling seemed to happen more than normal football games because of this game being easier. Also by sacking, you'll get the Not This Time achievement cause you'll likely win at least one blocking battle.

Also, by getting power-ups through out the game, you should easily be able to get 5 different ones (as they pop up randomly on the field and will look different). If you get and use 5 different ones, you'll get...
Now for Game 2. This one we will focus more on passing (you can run too obviously). Also, if you haven't already, try to return punts to the house (TD) as much as possible, purposely punting more, and purposely setting yourself up for a 50 yard field goal. Make sure you play it so this game goes into OT (don't worry, you can easily win). Empty Backfield, then Hail Mary is a good play option here.
Game 2
Also, try and get yourself backed up to the half yard line and complete a 100+ yard play (via a Hail Mary or Deep Pass play in general) to get...
After the first two games, you should have a bulk of the achievements of the game. After you beat story mode, which includes the 6 games acquiring your All Star team and win the 3 game tourney with your team and the end, you'll get...
Now for Tourney mode. There's only 6 different tournaments to go through and you can simply simulate through all of them until you win for the achievements to pop. Really only took me an hour to get all 6 and if you truly want to ensure you win, simulate until your team gets to the finals and quickly play the final game (1 minute quarters) and get the achievement. All that should be left now besides a few mop-up achievements that you may or may not have gotten already is Season Mode. You can simulate the season to get the Season mode specific achievements, however you want to make sure you win your first five games to get...

Now you'll have 4 more achievements to get (unless you got some of these while playing, which could very well be possible.)
Set up a regular exhibition game, 5 minute quarters, and have 2 controllers to control both teams. This will give you ample time to set yourself up to get these achievements in probably the easiest way possible. Choose a great team for your profile controller and a bad one for the second controller. Basically you want to set yourself up to intercept the ball and then run it back for a touchdown, purposely controlling the second controller to their half yard line to get a safety on them, and punting the ball 40+ yards and controlling the second controller to not return the ball at all. Pick a team with 5 in power like the 7th St Brawlers.
For the interception:
Profile controller: Use Cover, then Prevent defense. This will give you the best coverage to make a INT.
2nd: Shotgun, Flags.
Just select a defender, preferably the far right or left one, and hike the ball with the 2nd controller and right before the receiver gets to the ball (like 5-10 yards before), press the button of the receiver and quickly grab your profile controller and get to the ball and press . You should easily pick the ball off. Now just run it back with for sprint and juking defenders with and if you can dive extra players out of the way with the 2nd controller just in case.

For the safety:
With the 2nd controller, run them backwards to the 1-3 yard line at least. Now with your profile controller, select Pressure, then Blitz. Now hike the ball with the 2nd controller and quickly grab your profile controller and get to the QB in the endzone and hit to sack him and get your safety. You'll get...
Now for the final achievement. With the profile controller and your team having a 5 in power, punt the ball and then take control of the 2nd controller. Simply run backwards with the 2nd controller if need and simultaneously use push down on the joystick of the profile controller and once you got a defender near, tackle the 2nd controller player and you'll nab...
