In your own time after completing the contracts and side quests in Velen that you can, continue with the main quest and make the long journey to Novigrad and meet up with your good old friend, Triss – Pyres of Novigrad quest.
As always, go to the notice boards and get all the contracts. At the Heirach Square notice board, you’ll get the contract “An Elusive Thief”. Go through the usual motions of the contract, and eventually you’ll get to fight this particular ‘monster’. You’ll have the option to save it or kill it. It’s advisable to save it since you’ll receive a sizeable monetary reward and the diagram for the rather powerful sword, Gnomish Gwyhyr that you can use if you’re level 42! Whatever you choose, return to the merchant for your reward. He’ll pay you and you will unlock...
After you finish Trish’s main quest, Pyres of Novigrad, another one will unlock Broken flowers that will need you to locate an old dear friend. The quest will have you track down people who are likely to have info on the missing person. Just before you head off on your search, be sure to speak to Zoltan who should give you tips on everyone on the list. It will help especially with Rose van Attre. On your way to interrogate Rose van Attre, you’ll unlock a new notice board. Next to this notice board is a merchant here. Make sure you have at least 1,000 crowns and make a save before going in to interact with the merchant. In his wares, there’s a Potion of Clearance that you can use to clear all your abilities. Take the potion to clear your abilities, then use all your points in one tree, preferably the General branch that only needs 10 points. Once you do this you should unlock Dendrologist. Once you unlock the achievement, you can reload your save.
You should also buy A Miraculous Guide to Gwent Book from the same merchant for only 3 crowns. While it won't mark the locations for missing gwent cards, it will let you know how many are pending in each region of the map.
Keep on with the main quests and when you complete Count Reuven’s Treasure, a side quest will be available A Matter of Life of Death working with Triss. Be sure to do this quest if you’re going for the Card Collector achievement. It’s also an opportunity to get romantic with Triss. There are 3 gwent cards available here that would otherwise have been missable. Once you finally get to the ball, go to the (!) marker where there’s a gwent tournament going one. There’ll be 3 opponents and by beating all 3, you’ll get the Bruxa, Dandelion and Milva cards.
By now, you should have a pretty decent deck and, likely have 3 hero cards in your Northern Realms deck (likely Phillippa Eilhart, Yennefer of Vengerberg and Vernan Roche cards). Hero cards are the ones with black numbers, and aren't affected by special cards. As you’re continuing your gwent challenges, if by the luck of the draw and using spy cards you get 3 hero cards, save them for one round then use them all and hopefully win the round. Doing so will unlock the All In achievement.

As you progress with the main story, there’ll be 2 quests you’ll do in your search for Ciri; Count Reuven’s Treasure and Get Junior. A couple of quests will be available after a few cut-scenes and a story-related quest; Now or Never and Race: The Great Erasmus Vegelbud Memorial Derby. In Now or Never, you will continue where you left off with Triss, and completing this quest will also unlock another side quest, A Deadly Plot that you need to complete for another achievement later on in the game. Do the horse races as well in the build up to the Fast and Furious achievement. The races are different from those at Crow’s Perch as you don’t get to choose you competitor. The race is located close to the Vegelbud Residence.
In our patient build-up towards some of the cumulative achievements, equip the Dragon’s Dream bomb. If you don’t have this, you can purchase the diagram from the herbalist, just N.East of Oxenfurt. While you're here, if you haven't already unlocked Bombadier for collecting 6 different formulae for bombs, you can purchase any missing ones for this herbalist for the achievement.

Equip Igni and when you’re in a fight, use Igni and when the opponent is on fire, throw the Dragon’s Dream at it. This should cause an explosion. Do this 10 times and That is the Evilest Thing should unlock.
Once you’re done with this, equip the Devil’s Puffball bomb and also have Broadhead bolts equipped. You might have collected enough in your travels or you can easily craft some at a blacksmith. Keep Igni equipped as well. Now when you go into battles, throw the Devil’s Puffball and keep your distance. Once the enemy/enemies is poisoned, equip the crossbow and hit them with the Broadhead bolt which will induce bleeding. Now hit them with Igni before they die. Doing this 10 times will get you the Overkill achievement.
Make an opponent suffer from bleeding, poisoning and burning simultaneously. Do this 10 times.
After completing the Overkill achievement, you can go for Master Marksman achievement that's a bit grindy. You'll need to kill 50 humans and non-humans by killing them with a headshot. It helps to spend some ability points on Lightning Reflexes in Markmanship of the Combat tree and Trick Shot in the General tree can also help. You should also damage the enemy entirely with crossbow bolts before taking them down with the headshot. If they take damage from you signs or swords, even if you do kill them with a crossbow to the head, it seems not to count. Humans are the best for this, but . You can also use Axii to stun an enemy to make it easier getting the headshot kill. Now get 50 of these as you're carrying on with the quests and contracts and the Master Marksman achievement is yours.

After the Count Reuven’s Treasure main quest, a couple more will follow as you continue in your search. The next achievement is story-related to these two quests and will unlock when you complete Poet Under Pressure. You will unlock...
Go to the Rosemary and Thyme pub and Zoltan will engage you in conversation, letting you know of some troubles he has. Choose to help him, but just before you go on the quests, play Zoltan in a game of Gwent. Beat him and you’ll get a special card, Eithne. You also unlock other players you can play for rare cards in the Gwent: Old Pals quest. Continue on with the quest, and at the end you’ll have the choice to either take the gold or the cards. Naturally, if you’re going for the Collect em All achievement, keep the cards.
Next, you could be ready for the High Stakes quest with the set of cards you have now. Have 1,000 crowns in hand as well to enter the tournament. At the start, you will get chatty with Sasha who’ll give you a brief overview of some of the competitors. You’ll need to talk with them before the tournament begins. The tournament is a 3 round elimination before playing the final. In the first round, you’ll play the dwarf who shouldn’t be too hard to defeat. After the 1st round, Sasha will call you aside and give you another proposition. Accept and go in for the second round, which will be against Sasha. She’s tough playing as the Nilfgaardian faction and she’ll spam you with spy cards that will likely outnumber your cards. When you do defeat her, you’ll play the arrogant half-elf. He’s not too hard and you should be able to defeat him easily, which puts him in a foul mood and you get into a bout with him. After you defeat him, you will play against Tybalt in the final. He’s a decent opponent and uses Monsters deck, but you should manage. After you defeat him, Gwent Master will pop up.

If you chose to work with Sasha, the quest will continue and you’ll need to do some investigating and eventually track the thieves. At the end of this quest, there’s a chance to romance Sasha. You’ll know what to do when the time comes!
