The first chance you have to disable the vita-chambers is after the first cutscene once you have started a new game. There is no option to disable the vita-chambers at the main menu, it must be done once you've started a new game. Hope this helps clarify as I wondered the same thing.

You can also get the achievement by simply not dying. This has been confirmed by drachen77.

is it NECESSARY to actually disable vita-chambers from the menu to get this achievement, or can you get this by not dying in the game?

I would assume that you can still get the achievement if you don't die, even if the vita-chambers are still enabled. But that would just be an unconfirmed guess on my end. I would still recommend disabling the chambers as soon as possible, as dying once, even accidentally, would void the achievement.

I can confirm that if you just don't die, you can get the achievement. My son plays all games on Easy, and never died and got the achievement (falling over laughing from the name, by the way).
