I thought this achievement was broken or glitched after maxing out every aug on my new game plus play through. Turns out, typhoon was green but was sitting at 0/0. I had to select it and choose permanent lethal or non-lethal for it and then BAM achievement unlocked.
So, if you're stuck with the achievement not unlocking even though it looks like it should, check typhoon.
