Thank you for everyone's comments, Maka91 cleared up the issue in a comment
" If you press X at the mission select, and start the game on a new save slot under Veteran, you'll only need to complete 1 ending (Ashes to Ashes is easier). If you level select your way through a previous save, you'll need to do both ending if you have them both unlocked."
Full credit goes to him on this note, which also makes sense since I did Veteran on my first run through and just went back and did level select through the menu.

Thank you Maka91, never though I would ever get a comment on a solution from someone I watch and admire. toast Definitely a high point for me as an achievement hunter even if it's a basic guide on a pretty straightforward achievement.

Some have mentioned in the comments that they didn't need to complete Ashes to Ashes if they didn't chose that ending. For me I did then it popped so your mileage may vary

I would like to echo the other guides in saying it is to beat the campaign on Veteran.
But in order for the achievement to pop you must complete both missions, Ashes to Ashes and The Final Countdown. The choice for the different missions to either is found in the mission Identity Crisis.

Another side not is you DO NOT need to complete the side missions for this to unlock. I had only completed one (Red Circus) during my Veteran play through and still got the pop.

Hope this helps, Happy Hunting.
SoundgoodizerThanks for this heads up, I wouldn't have thought that you would need to do both endings for this, seems weird

SoundgoodizerActually I just did this and you don't need to complete both endings. I did this for my 2nd playthrough and I ended with Ashes to Ashes since I did The Final Countdown on my recruit playthrough. I was planning on going back and redoing The Final Countdown based on what you said, but the achievement popped upon completion of Ashes to Ashes

MushaConvoyIntending to do Veteran as my first playthru, can I replay Identity Crisis via mission select to choose the other outcome or need to replay from the beginning?
Posted by MushaConvoy on 14 Nov 20 at 08:37
hi3gie12You do not have to do both missions on veteran difficulty.
Posted by hi3gie12 on 14 Nov 20 at 09:31
Itzz Sh0wt1m3If you dont choose the dialogue option to unlock ashes to ashes, then you dont need to complete that mission

Arkinvader99Thanks all I made a note in my solution, I did Final Countdown and didn't get the unlock until I completed Ashes to Ashes on Veteran
Posted by Arkinvader99 on 14 Nov 20 at 18:01
Konvict VIVIVII also didn't have to complete both final missions to unlock this. I just did The Final Countdown and the achievement unlocked.

MushaConvoyJust posting back my results, chose Veteran on my first playthrough, chose Final Countdown to be my ending and unlocked at the end.

I did not have Ashes to Ashes unlocked and in my mission select at this point, this may be the cause for both missions needing completion. Certainly good to know if anyone finds it not unlocking.

KantaryaI had to complete ashes to ashes to get this achievement

CraKeLLChoose TRUTH and you only have to complete one ending

I Ebon Hawk IAchievement didn’t unlock for me until I did both side missions and then re-played Ashes to Ashes on Veteran. I did Ashes and Final Countdown on Veteran before that and it wasn’t unlocking for me.

CraKeLLDidn't complete either side mission or Ashes and got my cheevo

shadow walkerOZFor me, I had already finished the game on normal and telling the truth. When I played through on veteran, I chose to lie and the achievement popped for me without having to go back and play the other ending on veteran.

The Bartlez 2I played through on Veteran on a new save file and did not do the side missions and only did "Ashes to Ashes" and it popped at the start of the credits.

JJR0906What would happen if you did the majority of the mission on a low difficulty then upped it to Veteran right at the end? Does the game recognise that you haven't done the mission on Veteran
Posted by JJR0906 on 21 Nov 20 at 15:48
JJR0906Ignore my previous comment, the campaign isn't that difficult on Veteran. Managed to complete it all in a day. You just have to patient, and not rush through areas, make sure you wipe out all enemies before advancing. Surprised on how easy it was, perhaps Veteran got toned down because of the Realism difficulty

NebuleyI did all missions on veteran, and the achievement hasnt unlocked. I have the achievements for all other missions, just not this one. Any help?

SunseI did every mission on veteran and this hasn’t popped for me.
Posted by Sunse on 22 Nov 20 at 02:01
Nebuleyupdate: it didnt pop, and it still says locked in my achievements menu, but i do have the achievement. it shows up on trueachievements and in my activity feed. it may just be a bug but check and see if this also happened to you!

KnightSlasher83I did ashes to ashes on veteran and it popped for me without even doing the other mission idk why ot isn't working for everyone else I think it is because since I found the beginning of final countdown too hard I loaded back to the mission where you choose what ending and then I chose the lie one, I hope this helps

GTAmissions1Just did my Veteran run and unlocked it after Ashes to Ashes (lie). Doing a minimal run focusing on completing what is necessary.

No side missions and no need to replay the other one (truth) in my case. Not changing the difficulty if I needed to bring myself back to the main menu or safehouse to bring myself to the correct campaign level if it pushed me through to repeating one already done.

Kale VoorbipsJust completed veteran this afternoon, only completing the 'truth' option, so I guess only one of the choices needs to be completed for this achievement to pop. Having to do both looks like a glitch to me.
Veteran was actually a lot easier than expected. I did my first run on regular and some situations were actually quite hard to get past so I was expecting the worst for veteran, but it feels certain parts were actually easier to do.

kriegirDid my second play through on veteran. When I finished it didn't pop and a few random missions weren't showing veteran. Annoying. Anyways it unlocked after I beat them
Posted by kriegir on 21 Dec 20 at 04:02
GerbasgamerSo I just played a few missions out of order and I got the achievement despite not having completed all missions on Veteran.
It popped after finishing "The Final Countdown" but I haven't even finished "Desperate Measures" on Veteran. However, I did complete "Ashes to Ashes" but none of the side missions. If you do both endings, maybe you're allowed to skip one of the other main missions.

FancyKilerWalesThis has to be one of the easiest CoD campaigns on Veteran. Make sure you take the extra health and explosion resistance perks when building your character at the start. The are only two truly challenging parts, one where you are being extracted by a helicopter on a server (there is just no cover) and the other is a brief section in a car where you have to use a grenade launcher in the final mission (truth ending).

Rubber DucksssYeah pretty easy to complete on Veteran. I tool through perks Violent Tendencies (bullet damage increase 25%) and Survivor (health increase by 25%).
I did all side missions as well, but didn't have Ashes to Ashes unlocked and still got the achievement at the end of The Final Countdown

KDC720Definitely one of the easier CoD campaigns on Veteran difficulty. I did both side missions however the achievement popped for me as soon as I completed "The Final Countdown", I did not have to do "Ashes to Ashes".

I took Aggressive Behavior and Calm Under Pressure as perks for Bell, but I had very little difficulty overall. In general, I'd say try to remain undetected during stealth sections, and take advantage of hidden weapons. For example, The Redeemer revolver you can find on "Echoes of a Cold War" and "End of the Line" is a one hit kill and ammo is plentiful.

Maka91If you press X at the mission select, and start the game on a new save slot under Veteran, you'll only need to complete 1 ending (Ashes to Ashes is easier). If you level select your way through a previous save, you'll need to do both ending if you have them both unlocked.

Arkinvader99I have made a edit to my solution adding Maka91's addition, thank you everyone for the comments I hope this helps!
