When going through all the different paths in the story you get this on your third or fourth path to endgame. I got this one on my third path.
Start-->Choice 1--> Lighting Malfunction-->Choice 2--> Direct Route--> End.
Start-->Choice 1--> Lighting Malfunction-->Choice 2--> Inverted City--> End.
Start-->Choice 1--> Hostage Situation-->Choice 2--> Direct Route-->End.
Start-->Choice 1--> Hostage Situation-->Choice 2--> Spacewalk-->End.
