How to avoid the glitch that will prevent you from getting this quest.

- preferable method- Don't ask the Jarl in Winterhold about any work until you have beaten the Companions quest line. He will give you a quest to find the Helm of Winterhold which interferes with the 2nd to last quest in the companions quest line.

- if the quest is active and not yet complete - retrieve the Helm of Winterhold without killing the last 3 guys in the dungeon. I froze them with my dragon shout. Then turn in the helm before returning to the companions with the witches head.

Hope this helps.
Mazrael I'd already cleared the dungeon out before receiving the quest, and didn't have a save before it either.. Never heard the Helm quest until a few nights later - only option for me would be a second character that goes straight for the companions quests exclusively - the 1.2 patch had no effect on it

TheEnigmaShew wishing I had read this a week ago I completed that quest and had been avoiding the companion quest for a bit now

steveRPGmr From my experience with the Helm of Winterhold glitch, it seems to have been fixed with the 1.4 patch!

Disturbed XBA Got to go back to an older save myself. Got one of the Trouble in Skyrim quests to kill a leader who is already dead... This seriously needs sorting. Happens all across the game.

Grousta I just got the achievement yesterday after the game glitched on me. I cleared the area with the witches and took a head to the Companions. Then the Purity of Revenge quest did not start, because the Helm of Winterhold quest was active. I didn't realise it was a glitch back then. I complete the Helm of Winterhold quest, but nothing changed (I had no old saves). I just thought: this achievement is no longer obtainable, so I went on with the rest on the game. Yesterday (after a very long ingame time) I walked into Jorrvasker and the Purity of Revenge quest started!

I don't know if I was lucky or that it works for everyone, but I was very relieved. One think I did after I realised it was a glitch, is stop the mourning of the Companions by attacking one of them. Maybe this helped too, but this was done a long ingame time ago, so it does not help immediately.
