This is rewarded for solving all puzzles in the game. You cannot unlock this achievement, if you have skipped some of the minigames (sometimes you can do that by pressing RT)!! So it is very important, that you solve everything "on your own".

Again, I can recommend the guide by MaGtRo, which is excellent:

The achievement pops right after solving the last puzzle (you don't have to finish the game at this point anymore).

Note: As stated and confirmed by ugli, playing the missed / auto completed puzzle doesn't help. You have to get this in a single playthrough.

Reminder:
There are many people who confirmed this achievement to be buggy. As for now, there is no known solution on how to unlock it, if you failed to. My only recommendation in this situation: Though a lot of work: Play through the game again.

Does this carry on multiple playthrus.

If say, I miss the first puzzle (accidentally press the RT and realize it later) do I have to play the game second time till the last puzzle or will this pop after I finnish the first puzzle on second playthru?

I'm not sure, but I think you have to play the game all over again. But needs confirmation.

This achievement is actually buggy. It never unlocked for me, so I Googled it and several players across Xbox and PS3 haven't unlocked it when they should have.

I can confirm that you need to play to whole thing over again. Playing the missed / auto completed puzzle doesn't help. Dunno about the bug tho.

Didn't unlock for me. Neither did the lockpicker achievement. When I've completed a 2nd playthrough and hopefully it unlocks ill update with what I did different... If anything.

I didn't skip anything, but this one didn't pop for me; everything else did. I'm starting over now. I'm wondering if it's because I backed out of a puzzles a few times before they were solved, saved, quit the game, and then picked up from there the next time. Not sure what else it could have been.

I played it through again using a guide to be as efficient as possible--just took a few hours--and it popped for me after the last puzzle, so you don't even have to finish the full game.

Unlocked fine for me the first time. Honestly if you have to play it again, though, you could probably skip all the dialogue and cut the game time by 80%.

I'm fairly sure you can't skip a puzzle OR use your "sixth sense" ability by pressing LT. Either could void it. Anyone confirm my theory?
